Consumers are also eligible to a wide array of services through the home and community-based program including: Specialized Therapies, Day Habilitations, Dental treatment, physicians & physical therapies, Transition Assistance, Minor Home Modifications, and Adaptive aids. These services are rendered as needed based on the individual plan of care.
Specialized Therapies
These includes Behavioral Support Services, which provides specialized interventions that assist individual to reduce the frequency, intensity, and duration of challenging behaviors and manage co-occurring mental health issues. These services are offered through contracting with licensed professionals and are rendered as needed.
Day Habilitations
These services include activities to help individuals become more independent in their daily activities by providing out of home learning to acquire, retain, socialization, and increase their adaptive skills necessary to live successfully in the community. we will contract with your choice of licensed day- habilitation providers to provide the desired services.
Dental treatment
Dental care is the maintenance of healthy teeth for the individual and the practice of keeping the mouth and teeth clean to prevent dental disorders. The professional care of the teeth is being provided by a dentist and are rendered to the individual as needed. These services are offered to the individual through contracting with licensed professionals
Physicians & physical therapies
These services are provided to the individual by a person who is qualified and specializes in diagnosis and the treatment of disease, injury, or deformity by physical methods. These treatments are available and are provided as needed to meet individual needs.
Transition Assistance
Transition support is provided for those individuals who need support and information in the development of a transition plan that will better prepare them to move out and into the appropriate adult services.
Minor Home Modifications, and Adaptive aids
This allows a repair and maintenance of a minor home modification and assistive devices to increased personal accessibility and independence of an individual residence.
